Family Care Southwest wants our patients to have access to their medical records anytime from anywhere. In order to deal with the complexity of modern medical care, patients need to be able to view their medical records from anywhere in the world. We are now providing this service with a password secure access from our website.

PATIENT REQUESTS ALL-IN-ONE FORM

Patients may register for their own, secure, on-line account. Those who sign up will have access to EKGs, X-ray reports and lab studies.

Once you Register and Login to your secure online account, all Patient Requests can be made from one location. You will see a list of file names for any Medical Records available for viewing. You will also be able to Make an Appointment, send Secure Mail to your Doctor and request a Prescription Refill -- All from the same form!

Electronic medical records are very portable within minutes they can be sent to emergency rooms, insurance companies, or specialists to expedite patients’ care.

Having internet access allows Family Care Southwest to interface with specialized medical services like Colorado Heart and Body Imaging and Invision. They are able to send us X-rays, CAT scans, and MRls with digital images which we can view in our exam rooms.

Register for our on-line patient services for secure access to your EKGs, X-ray reports and Lab studies. You can also make an Appointment, request a Prescription refill and send a message to your doctor with Secure Mail.

FULL MEDICAL RECORD ONLINE $19:

You get a complete copy of your electronic medical records on line. Note that you must already be signed up for FREE Medical Records Online in order to view the FULL Medical Record. Note: your paper chart will have information prior to the Practice going onto electronic medical records that will not be uploaded.
